We need proof (doctor's signature or copy) that the student has had their measles, mumps, and rubella two shot series. Students planning to live on campus will also need to provide proof of vaccination for meningitis. You can find the formin the back of your acceptance packet or online at Immunization.
- Citizenship and Residency
If you receive a letter indicating you need to complete a citizenship or residency form, please do so and return it to the appropriate office. If you are trying to claim in-state residency, but live in Richmond or Columbia County, Georgia, you must complete the Richmond/Columbia County Residency Form. For all other citizenship and residency forms, visit Citizenship and Residency.
